A new care service to help people in the western suburbs choose the best support options for them, has begun.
The Health Care Finder Service started last week and is designed to assist the elderly with finding services and supports that help with every day tasks.
Operated by IPC Health, the service is available to elderly residents in the municipalities of Wyndham,Maribyrnong, Hobsons Bay, Brimbank or Melton, who are eligible for aged care services.
The service provider can also benefit people who might be isolated and don’t have a family or friend to help as well as those who have difficulty communicating and reading and writing.
Those who generally find it difficult to understand information and make decisions or are reluctant to engage with aged care support or other government services can also benefit from Care Finder Service.
The Care Finders at the service can speak a range of languages , and in the instance an interpreter is needed it will be free and available.
IPC Health chief executive Jayne Nelson said IPC health is deeply embedded in western and outer western Melbourne as they continue to assist older, diverse and vulnerable communities to navigate and access aged care.
“Our Care Finders can help older people who have challenges or barriers accessing and arranging aged care services and supports, like My Aged Care and who don’t have a family member, friend or other representative who can help,” she said.
“IPC Health’s Care Finder Service is a free service and supports a range of older people, including people from Culturally and Linguistically Diverse communities and Aboriginal and Torres Strait Islanders and those living with a disability.”
